5/6 grade Health Info
As a reminder to parents/guardians who have a child with a known Life-Threatening Allergy and/or Asthma. In an effort to have your child in compliance with the District Medication Policy at the beginning of the 2015-2016 school year, please refer to the information below.

LIFE-THREATENING ALLERGY
If your child requires an EpiPen for the treatment of a known Life-Threatening Allergy, it is the parent’s/guardian’s responsibility to provide the school nurse with the EpiPen and Anaphylaxis Action Plan.  This form can be found on the District Website.

ASTHMA
If your child requires medication to treat his/her Asthma please have your child’s physician complete the Asthma Inhaler Permission form. This form can be found on the District Website.

Administration Of Medication During School Hours-
Medication should be scheduled around school hours if possible. Medications orders are required from a physician for prescription and over-the-counter medicine, as well as herbal remedies.
All medications administered in school require the completion of the “Authorization for School Medication Administration” form found on the district website under Forms, Health. A new prescription is needed every school year for medicine that is taken on a long term basis. No medication will be administered by the nurse without proper physicians orders on file.  Physicians orders for the school year 2015-2016 must be dated after July 1, 2015.
